For patients who no longer require hospitalization but are not ready for full independence, “SNF at home” may offer patients a better alternative to skilled nursing facilities—receiving care in the comfort of their own homes.

This service allows patients to receive care typically provided in a skilled nursing facility within their home, with support from caregiver visits and remote monitoring technology.
