Application Process
PMHNP graduates, new hires and students who seek to gain valuable community health experience are encouraged to apply. The online application is confidential and takes only minutes to complete. In addition to the application itself, the following documents will be required for upload:
All applicants must provide the following:
- A copy of your Nursing License (All applicants)
- A brief personal statement describing your professional goals related to seeking a fellowship with a community health center (Max: 350 words).
PMHNP Graduates, including newly hired graduates must also include the following:
- Copy of your CV or resume
- Copy of your Nurse Practitioner Certification and NPI number if available
- Official Transcript (can be in process)
- MA Controlled Substance License/DEA or invitation to sit for licensure
- Two letters of recommendation (graduates only, professional and/or academic)
- PMHNP Students must provide a Letter of Endorsement from your clinical placement representative.
- All interested individuals must apply. Once your application is submitted, UMass Chan GSN will review your information, determine eligibility, and match you to a community health center (CHC).
Following CHC matching it is expected that Fellow candidate interview within a timely manner to determine best fit for Fellow and the CHC. No Fellowship placement is guaranteed.
