Away Rotation Health Forms Guide

As you prepare to apply to away rotations, please use this guide for helpful tips on how best to complete away rotation health forms and coordinate with Student Health Services for completion.

  • SHS will assist with completing away rotation forms/providing copies of immunizations and titers required by the rotation and reviewing the forms to let the student know what additional requirements are needed. 
  • Student Health cannot complete/sign off on your forms until all requirements are met.  
  • Please plan several weeks ahead to complete rotation requirements as you may need to schedule appointments for a physical, drug screening, fit test or other necessary appointments. 
  • Please review the rotation forms carefully before sending as institutions may have requirements that are different than the requirements UMass Chan had when you matriculated. They may require you to have additional bloodwork or vaccines. 
  • Please e-mail Studenthealth@umassmemorial.org and attach all the application forms for the away rotations you intend to apply. This allows them to all be efficiently completed together. 
  • If a physical is required, you will need to contact your PCP’s office. If you do not have a PCP you can go to a walk-in clinic for a school physical. (The SHS website lists some sites in the area.) 
  • If drug testing is required, contact UMass Memorial Health Employee Health Services at (508)793-6400. 
  • If you require vision screening or color-blind testing, please contact your ophthalmologist or optometrist. If you do not have one, you can contact your insurance company to identify optometrists in the area that your vision plan covers. 
  • If the form requires attestation of your health or mental health, you will need to obtain this from your own provider or a provider that completed your physical. 
  • Please take note of the Quantiferon Gold / TB requirement. Some sites require specific timing (i.e., within 3 months of starting the rotation). Please ensure the Quantiferon Gold timing satisfies all sites’ requirements vs having to have repeat labs a few months later. (The Quantiferon Gold can take up to 10 days to result.) 

Plan ahead for application deadlines. Allow 7 business days for SHS to complete your forms.

  • Consider the additional time required if you need vaccines or additional labs. 
  • If you have vaccines or titers completed elsewhere, please provide a copy to SHS for your record. 
  • Students are responsible for any costs related to health requirements for away rotations that are not covered by insurance, i.e., copays, deductibles and balances.

SHS will scan and e-mail the completed forms to your UMass Chan e-mail.
