Duplicate Diploma

Requests for a duplicate diploma must be made in writing and include:

• Signature of the graduate
• All names used on university records (last, first, middle, maiden, other name)
• Year of birth
• Last 4 digits of Social security number
• Telephone Number
• Email address
• Complete address for mailing of diploma
• Check in the amount of $53.00 payable to Miles Kedex Company, Inc.
