Moonlighting Information
A FULL license is REQUIRED.
In order to moonlight OFF-CAMPUS, you MUST have both Federal and MA DEAs.
You MUST fill out a
2012-2013 AUTHORIZATION FORM FOR RESIDENT/FELLOW TO RECEIVE ADDITIONAL COMPENSATION FOR EXTENDED EMPLOYMENT
. One form per site must be completed. Please make sure your program director signs the form before returning it to the Office of Graduate Medical Education (OGME) in the Medical School, Room S2-332.

WHITE EXTRA COMPENSATION REQUEST FORM
is for IN HOUSE ONLY and the
BLUE EXTRA COMPENSATION REQUEST FORM
is for EXTERNAL MOONLIGHTING ONLY
If you are moonlighting in the hospital (UMass & Memorial), you may use the DEA the OGME has provided for you. Please note – you have 3 months to apply for your Federal and MA DEA registration and by 6 months you should have your certificates. Once you receive your certificates, please provide our office with copies of both.
