Student Health Clearance Policy
UMass Chan Medical School requires all students admitted to the T.H. Chan School of Medicine, the Morningside Graduate School of Biomedical Sciences or the Tan Chingfen Graduate School of Nursing to complete the Student Health Clearance process prior to matriculation. This policy was developed to decrease risk of transmission of potential infectious agents, and it is part of each student’s professional responsibility to maintain this clearance throughout his/her educational experience at UMass Chan.
Procedures
Forms that must be completed can be downloaded from the Student Health Services (SHS) website using the following link, and must be returned no later than one month prior to the start of classes to:

The Student Health Services (SHS) office can help with any questions regarding the clearance paperwork and can assist with resources to help complete the process.
Consequences of Non-compliance
Entering students: Student health clearance is mandatory and forms must be completed and submitted prior to the start of classes. Non-compliance will result in revocation of the offer of admission.
Returning students: Student health clearance is required for attendance at classes and participation in clinical or laboratory activities. Non-compliance will result in suspension of registration and may result in a report of professionalism concerns and consequences up to and including dismissal. If a student is notified by SHS that health clearance records are incomplete, they must contact SHS immediately for assistance in resolving the problem - see SHS web site above, or call them at (508) 334-8464.
